The health declaration form provides a record of each passenger’s current health situation. You must state if you are showing signs of COVID-19 symptoms, or if you have been in contact with anyone showing symptoms.
This allows Puerto Rican authorities to reduce the risk of visitors carrying the virus into the country.
Each applicant must also provide their contact details for when they are in Puerto Rico. This information makes it easier and quicker for health officials to contact a passenger if they have been in close contact with someone that has tested positive for the virus.
The Travel Declaration Form for Puerto Rico is an additional way to make sure that all visitors are following the correct procedures set by the Puerto Rican government.
The current policy requires passengers to state if they have been fully vaccinated. Those who have not received a full dose of either the Moderna, Pfizer, or Janssen vaccine must show a negative molecular or antigen test result, taken within 72 hours of arrival.
