Usually you will be asked to show the health declaration form at check-in, boarding gates, and immigration.

All passengers must take this mandatory step to ensure they are following the current COVID-19 procedures.

How does the travel health form work?

The health declaration form provides a record of each passenger’s current health situation. You must state if you are showing signs of COVID-19 symptoms, or if you have been in contact with anyone showing symptoms.

Do children need to complete a travel health form?

The Puerto Rico Travel Declaration Form is a mandatory requirement for anyone over the age of 2.

Children should be added to an accompanying adult’s form. There is an option for this on the application.

Puerto Rico Travel Declaration Form Online Registration

The online application for the travel health form is quick and straightforward to complete, taking a matter of minutes.

The form can either be completed ahead of travel, or upon arrival in Puerto Rico. It is highly recommended that you obtain the travel document before departure, to avoid delays once you arrive.

You will not be allowed to pass through border control until you have a completed Puerto Rico travel health form to show to immigration officials.

  • Do I need to be fully vaccinated to visit Puerto Rico?

    Currently, you do not need to be fully vaccinated to enter Puerto Rico.

    However, all international passengers who have not been vaccinated with either the Moderna, Pfizer, or Janssen vaccine must have a negative COVID-19 molecular or antigen test result to be allowed to enter.

    The test must be taken within 72 hours of arrival to Puerto Rico.

  • Do I need a PCR test to enter Puerto Rico?

    If you have been fully vaccinated against COVID-19 with the Moderna, Pfizer, or Janssen vaccine, then you do not need a negative test result to enter Puerto Rico.

    If you have not been fully vaccinated with one of these accepted vaccines, you must show a negative molecular or antigen test result. The test must have been taken within 72 hours of arrival.

  • Do you have to quarantine on arrival in Puerto Rico?

    There is currently no mandatory quarantine rule for visitors entering Puerto Rico.

    However, all passengers must be either fully vaccinated with an accepted vaccine, or show a negative COVID-19 test result.

  • Is the Puerto Rico Travel Declaration Form mandatory?

    The Puerto Rico health declaration form is a mandatory requirement for any international passenger wishing to enter Puerto Rico.

    Without this form, you will not be allowed to pass through immigration.
