The Puerto Rico Travel Declaration Form is a mandatory requirement for anyone over the age of 2.

Children should be added to an accompanying adult’s form. There is an option for this on the application.

Puerto Rico Travel Declaration Form Online Registration

The online application for the travel health form is quick and straightforward to complete, taking a matter of minutes.

The form can either be completed ahead of travel, or upon arrival in Puerto Rico. It is highly recommended that you obtain the travel document before departure, to avoid delays once you arrive.

You will not be allowed to pass through border control until you have a completed Puerto Rico travel health form to show to immigration officials.

    Currently, you do not need to be fully vaccinated to enter Puerto Rico.
