The travel document is specifically for use during the COVID-19 pandemic. It helps Filipino health authorities to manage coronavirus cases and protect public health.

Other countries have introduced a similar document. These new policies are in place indefinitely, and are in addition to all standard entry requirements.

What is the One Health Pass for the Philippines?

The One Health Pass is a type of Travel Health Certificate. The document is a way for the Filipino government to monitor the health status of overseas visitors, and minimize the risk of COVID-19 transmission. 

The Philippines travel health form is a mandatory entry requirement for all international visitors. Without an approved certificate, immigration officials will not allow you to enter the country.

This COVID-19 policy is an extra obligation for travelers and does not replace any other required documentation. Passengers must still show all standard paperwork to enter the Philippines, such as a visa.

How does the health pass work?

The Philippines One Health Pass is a way for health officials to screen overseas passengers before arrival.

You must state if you have any COVID-19 symptoms, or if you have been with someone who has tested positive for the virus. This makes it easier for authorities to identify and minimize possible coronavirus cases.

The accommodation details provided on the form allow Filipino authorities to know where each passenger is staying during their visit.

The details provided help the Filipino government to monitor quarantine requirements, which are currently in place for all international arrivals.

Each applicant must provide their contact details on the travel health form.

By providing your contact information, health officials can easily inform you if you are at risk of infection. For example, if you have been in close contact with someone who has tested positive for COVID-19, authorities can let you know quickly.

Who Needs to Complete the Philippines One Health Pass?

All international arrivals must complete the health declaration form, with no exceptions.

If you are planning to enter the Philippines for any reason, and for any length of time, you must follow this entry policy.

Failure to show an approved One Health Pass could result in refused entry.

Do residents and citizens of the Philippines need a travel authorization?

Citizens and residents of the Philippines are also obliged to show an approved One Health Pass.

Even with a resident permit, or a Filipino passport, passengers must have this additional travel document. 

Do children need to complete a travel health form?

Children of all ages also need their own Philippines health declaration form.

There is no option to group applications together, or add children to an existing form. Children must have their own separate document.

Adults are permitted to apply for the One Health Pass on behalf of accompanying minors, if they cannot complete it themselves.

Philippines Health Declaration Form Requirements

In order to apply for the One Health Pass, you need to have the following:

  • Contact information
  • Passport details
  • Travel information
  • Travel history (last 30 days)
  • Quarantine accommodation

Your accommodation must be a government-approved quarantine facility.

Entry restrictions can change at short notice. International travelers should check the most recent coronavirus travel information to ensure they meet all requirements.

Philippines One Health Pass Online Registration

The online application for the Philippines travel health form is quick and easy to complete.

Providing you have all of the above prepared, the process takes a matter of minutes.

The One Health Pass must be completed within 3 days of traveling to the Philippines. The system will not allow you to continue if your travel date is more than 3 days away.

To open the application form, you first need to select the following:

  • Date of arrival in the Philippines
  • Type of port of entry (air or sea)
  • Foreign national or Overseas Filipino Worker (OFW)

Once you have submitted your answers, you will be taken to the online form.

Health Declaration Form Application

To start the application for the One Health Pass for the Philippines, you need to provide personal information, such as:

  • Full name
  • Date of birth
  • Nationality
  • Passport number
  • Email address

The form then asks you for details covering:

  • Accommodation address in the Philippines
  • Travel details

Once these fields have been completed, you are asked to provide some COVID-19 related information. This covers:

  • Vaccination status and details
  • Travel history for last 30 days
  • Exposure to coronavirus
  • Symptoms

Before submitting the form, you must click that the information you have provided is accurate and true. Giving false information could result in legal action.

When you have completed the form, you will receive an email with the approved Philippines travel health form. The document has a unique QR code which you must show to officials during your trip.

If you need to update your information or access the online form again, you can use the transaction number which the form generates.

It is important to remember that the Philippines One Health Pass is an additional entry requirement during COVID-19. You must still show all other required documentation to travel to the Philippines.

  • Do I need to take a COVID-19 test to enter the Philippines?

    Yes, all passengers must show proof of a negative PCR test result to enter the Philippines.

    The test must be taken within 72 hours of departure and must be an approved PCR test. Other tests, such as lateral flow tests, will not be accepted.

  • Do I have to quarantine on arrival in the Philippines?

    Yes, all international arrivals must quarantine on arrival in the Philippines for 14 days. The quarantine must be carried out at a government-approved facility.

    Travelers will be tested for COVID-19 during the quarantine period. Vaccinated travelers will be tested on day 5, and unvaccinated travelers on day 7.

    Providing the test result is negative, you can check out of the government facility and spend the remaining days of quarantine at home.

  • Do I have to be vaccinated to travel to the Philippines?

    No, you do not have to be vaccinated to travel to the Philippines.

    However, the mandatory quarantine rules are different depending on your vaccination status.

    If you are fully vaccinated and your COVID-19 test is negative, you can leave the quarantine facility on day 5. If you are unvaccinated, or partially vaccinated, this policy is extended to day 7.
