Without the Kenya Health Surveillance Form, you could be refused entry into the country.

How Is the Information Used?

The information that is collected on the Kenya Travelers Health Surveillance Form acts as a declaration of health for each passenger.

As well as personal details, each passenger must also declare on the form that they have tested negative in a COVID-19 PCR test.

This is to minimize the chances of overseas visitors carrying the virus into the country.

Public health authorities then use the data to monitor and control the spread of the virus.

Submitting the information also makes it easier for Kenyan health authorities to contact passengers if they have been in contact with someone who has tested positive.

Who Needs to Complete the Kenya Travelers Health Surveillance Form?

All international visitors to all ports of entry in Kenya must complete the health declaration form before departure.

Even if you are just planning to transit through the country, you must still complete the form before you travel.

A separate form must be completed for each individual traveler.

Do Children Need to Complete the Form?

A separate form must be completed for each passenger, regardless of their age, along with all the required information.

There is no option to include children on an accompanying adult’s form. Each child must have their own document completed and ready to show at border control.

Who Is Exempt from Completing the Form?

There are no exceptions to who must complete the health declaration form to enter the country.

Regardless of the reason, if you are going to enter Kenya under any circumstances, the form must be completed and shown to officials when requested.

Health Declaration Form Requirements

Before completing the Kenya Travel Health Form, you must ensure that you are currently permitted to enter the country.

In light of the COVID-19 pandemic, it is always advisable to check current travel restrictions before you organize your travel.

Before you start the process of completing your travel health form, you will need to have:

  • A valid passport
  • A phone number
  • An email address (to receive your completed form)
  • Confirmed travel

In accordance with Kenya’s COVID-19 protocol, you will also need a negative PCR test result taken within 96 hours of departure.

Children under 5 years old are exempt from the test requirement.

It is important to remember that the test must be an approved PCR test. An antigen test will not be sufficient to enter the country.

The travel health form will ask you to provide information such as:

  • Personal information (name, nationality, passport number, etc.)
  • Your country of origin
  • Travel information (flight number, airline, seat number, etc.)

If you are traveling to Kenya by plane, it is advised to check in online before completing the form so that you have your seat number already assigned to you.

You will then be asked to declare that you have a negative COVID-19 PCR test result.

    Yes, you need to take a PCR test within 96 hours of departure for Kenya that shows a negative test result.

    Currently, only PCR tests are approved by the Kenyan health authorities. An antigen test will not be sufficient to travel to Kenya.

  • Do you have to quarantine when you arrive in Kenya?

    Currently, visitors to Kenya are not subject to quarantine on arrival.

    However, if a passenger fails to show a negative PCR test result, they will be refused entry, or face mandatory quarantine.

    Yes, the travel document is mandatory for all visitors to Kenya.
