The authorization is a specific COVID-19 document that must be obtained before travel, and then shown to officials during your journey to Jamaica.

What is the Jamaica Travel Authorization Form?

The Travel Authorization Form for Jamaica is a mandatory document that all international passengers must have.

If you cannot show this compulsory form to officials, you cannot travel to Jamaica.

Many countries now have a similar entry requirement, which acts as a type of Travel Health Certificate for overseas travelers.

The form must be completed alongside all other documentation that is required to travel to Jamaica. It is a COVID-19 specific policy, and does not replace other paperwork, such as a visa.

Health screen passengers

The Jamaican health declaration form is a way for authorities to health screen international passengers entering Jamaica.

Applicants must provide information related to their current health status. This helps officials to assess if a passenger is in danger of carrying coronavirus, and therefore too high-risk to enter the country.

Manage COVID-19 restrictions

The Travel Authorization also acts as an agreement between the passenger and the Jamaican government, stating that they will follow all COVID-19 restrictions during their stay.

Current requirements include health screening on arrival and a negative coronavirus test result.

Do Residents and Citizens of Jamaica Need a Travel Authorization?

All passengers, including citizens and residents, must apply for an authorization before they travel.

Without a valid travel health form, you do not have permission to enter Jamaica.

Do children need to complete a travel health form?

Children of all ages also need to have an approved Travel Authorization for Jamaica.

There is no option to add children to an existing application, so a separate form must be completed for each child. 

Accompanying adults have permission to apply for the travel health form on behalf of children who are too young to do so. At the start of the application, you must select that you are completing the form on behalf of someone else.

Jamaica Health Declaration Form Requirements

Before you start the application process for the Jamaica travel health form, you should ensure that you have the following:

  • Accommodation in Jamaica

It is important to remember that you must still obtain all other standard documentation that is required by the Jamaican government.

An approved Travel Authorization will only give you permission to enter Jamaica if you have followed all other entry policies.

Jamaica Travel Authorization Online Registration

The online application process for the Jamaica health declaration form is quick and easy to complete.

You must apply for the Travel Authorization within 7 days of your trip to Jamaica. The system will not allow you to apply more than 7 days before, so you must wait until you are within this timescale.

Before you can complete your application, you must first register your email address. A password is sent to this email address, which you must then input into the system to confirm your details.

The form starts by asking for some basic personal information, including:

  • Name
  • Permanent address
  • Date of birth
  • Phone number
  • Passport details

You must also provide a list of countries that you have visited in the last 6 weeks, and indicate if you are traveling with any family members.

Applicants must answer some health related questions, covering COVID-19 symptoms and vaccination status, before completing the form with your accommodation address in Jamaica.

Once the form has been completed and submitted, you will receive an email notifying you if your application has been approved. If it has, you will also receive the completed Jamaica Travel Authorization by email.

The document can either be printed or shown on a mobile device during your journey.

How Long is the Travel Authorization Valid for?

For travelers from designated high-risk areas, the health declaration form is valid for 10 days. 

For travelers from low risk areas, the form is valid for 7 days.

  • Do I need to be vaccinated to travel to Jamaica?

    No, vaccination is currently not an entry requirement for Jamaica.

    However, if you are not fully vaccinated, you must quarantine on arrival for 14 days.

    Yes, children of all ages also need an approved Travel Authorization to enter Jamaica.

    The quarantine can be completed at home, and must be for 14 days.
