Indonesia has introduced an additional travel health document for international visitors  — the Indonesia Health Alert Card (eHAC).

The purpose of the document is to support authorities and protect public health during the COVID-19 pandemic. It forms part of the special entry requirements and restrictions specifically for coronavirus.

Many other countries now have a similar document, which helps to safeguard foreign visitors and residents.

The Indonesia travel health form is a requirement for anyone visiting the country. Without a completed document, you could be refused entry.

What is the Indonesia eHAC?

The Electronic Health Alert Card is a health declaration form for international passengers. It is an additional document which must be completed along with all other required paperwork, such as a visa.

Passengers should apply online for the Indonesia travel health form before travel. The approved document must then be shown to officials when boarding, and again at immigration.

The eHAC is mandatory for all overseas visitors. You cannot travel to Indonesia without this travel document.

How does the Health Alert Card work?

The information provided on the online form works in various ways to help Indonesian authorities control the spread of COVID-19.

You must state if you have been in contact with someone who has tested positive for coronavirus, as well as providing your recent travel history. This allows health authorities to identify possible carriers of the virus entering the country.

Interrupt chains of transmission

The Indonesia eHAC acts as a point of contact between officials and each passenger. If you have been at risk of infection— for example, a fellow passenger has tested positive—you can be contacted quickly and informed of the necessary steps.

You must provide information related to Indonesia’s current travel restrictions, to confirm that you meet all requirements. It is important to check the most recent information to make sure that you have all required paperwork.

Who Needs to Complete the Indonesia Electronic Health Alert Card?

All international visitors to Indonesia must have a completed health alert card. The travel document is a mandatory entry requirement, with no exceptions.

If you intend to enter the country for any reason, you need the Indonesia health declaration form. This rule does not change depending on the purpose of your visit or length of stay.

Do children need to complete a travel health form?

Children need to also be included on the Indonesia eHAC. They do not need a separate form, as there is an option to add them to an existing document.

When you are in the process of completing your form, click “add more applicants” to add any accompanying children to your request.

Indonesia Health Declaration Form Requirements

Before you can start the application process for the eHAC, you will need to have the following:

  • Contact details
  • Travel information
  • Accommodation in Indonesia

In line with Indonesia’s current COVID-19 safety measures, you must also have:

  • Negative COVID-19 PCR test result
  • Proof of full vaccination

You must register your vaccination certificate on the Indonesian government website before you travel.

There is currently a mandatory 10-day quarantine for all foreign visitors. This is increased to 14 days for anyone who has been in a high-risk country.

It is important to check the most up-to-date entry requirements, as policies can change at short notice.

Indonesia Health Alert Card Online Registration

You must complete the eHAC before starting your journey to Indonesia. It is recommended to start the process well in advance of travel, to avoid any delays.

The eHAC can be completed through the government website, or by downloading the app. It is easier and more efficient to use the app if possible.

Before starting your application, you need to register online to open an account. You then use the same login details to complete the Indonesia eHAC.

The form only takes a few minutes to complete. First, you must provide basic information covering:

  • Personal information
  • Contact details
  • Accommodation address in Indonesia

You then need to give some details specifically related to COVID-19, such as:

  • Recent travel history
  • Current health statement
  • COVID-19 health certificates

If you are traveling with children, you can add them to your form by clicking “add applicants”. You will need to provide the details for each child on their behalf. Ensure that they are with the relevant adult when they present the form to officials.

When all the required fields have been completed, you can submit your application. The approved Indonesia Health Alert Card is then sent to you by email. You must show the QR code in the email so that it can be scanned.

  • Do I have to be vaccinated to travel to Indonesia?

    Currently, all international visitors to Indonesia must be fully vaccinated to enter.

    You must register your vaccine certificate through the Indonesian government website before you travel.

    If you have not been fully vaccinated with a vaccine approved by the Indonesian government, you cannot enter the country.

  • Do I have to quarantine when I arrive in Indonesia?

    All overseas passengers must quarantine for 10 days on arrival, even if they are fully vaccinated.

    Passengers arriving from certain high-risk countries must quarantine for 14 days.

    A PCR test must be done on arrival, and another PCR test must be taken 1 day before quarantine ends.

    All tests must be negative to leave quarantine.

  • Do I still need a visa to travel to Indonesia?

    All visitors who are required to have a visa for Indonesia must still show one. The Indonesia eHAC does not replace a visa. All standard entry requirements must still be followed, in addition to the Indonesia Health Alert Card.

  • Do I need a COVID-19 test to enter Indonesia?

    All international arrivals must show a negative COVID-19 PCR test result, taken no more than 72 hours before departure.

    Another PCR test must be taken on arrival, and again on the day before quarantine ends.

    The tests must be a PCR. Other COVID-19 tests, such as antigens, will not be accepted.
