Costa Rica has introduced a travel health form for passengers arriving in the country during the COVID-19 pandemic. This Pase de Salud”, or Health Pass must be obtained online.

The Costa Rica Health Pass is one of the measures the country has taken to combat the spread of coronavirus. It ensures that travelers are either vaccinated or have travel insurance to cover any COVID-19 treatment and accommodation required during the stay.

It also prevents individuals with COVID-19 symptoms from entering the country.

Who Needs to Complete the Costa Rica Health Pass?

All passengers planning to travel to Costa Rica must fill in a Health Pass form. This includes the following travelers:

  • Tourists
  • Business visitors
  • Costa Rican nationals
  • Residents of Costa Rica

A separate Costa Rica health declaration form must be filled in for each person traveling to the country.

Do children need to complete a Costa Rica travel health form?

Children must have a Health Pass to enter Costa Rica, just like any other traveler.

However, minors do not have to complete the Costa Rica travel health form themselves. Their legal guardian should fill it in on their behalf, according to the Costa Rican Ministry of Health.

The same applies to other dependents, such as those with a disability and elderly people who are not able to complete the form themselves.

Costa Rica Health Declaration Form Requirements

The Costa Rica Health Pass must be obtained before traveling to the country. This is done by filling in a mandatory online form.

In order to complete the form and be granted a Health Pass for Costa Rica, the individual must meet certain requirements.

The traveler must not be experiencing any symptoms of COVID-19, such as:

  • Fever
  • Cough
  • Loss of smell
  • Loss of taste

Foreign nationals must also meet one of the following requirements:

  • Full vaccination against COVID-19
  • Insurance policy covering medical expenses and accommodation due to coronavirus

Children under 18 are exempt from needing either a vaccination or insurance.

Vaccination requirements for the Costa Rica Health Pass

Vaccinated travelers must meet certain criteria to avoid having to get an insurance policy. They must:

  • Have received an approved COVID-19 vaccine
  • Have received the last dose of the vaccine at least 14 days before the trip
  • Have a vaccination certificate as proof

Approved COVID-19 vaccines for Costa Rica include the following:

  • AstraZeneca (AZD1222 / Vaxzevria / Covishield, ChAdOx1_nCoV19)
  • Johnson & Johnson (Janssen Pharmaceuticals / Ad26.COV2.S)
  • Moderna (Spikevax / mRNA-1273)
  • Pfizer & BioNTech (Comirnaty / BNT162b2 / Tozinameran)

Insurance requirements for the Costa Rica Health Pass

Travelers who do not meet the vaccine requirements outlined above must have a valid insurance policy to get the Costa Rica Health Pass.

They must bring proof in the form of a certificate from the insurer in Spanish or English. This must have the following details:

  • Traveler’s name
  • Validity of the insurance policy
  • Guarantee of coverage for at least USD $50,000 (or equivalent) for medical expenses related to COVID-19
  • Minimum coverage of USD $2,000 for additional accommodation required due to quarantine

What Is the Costa Rica Health Pass?

The Costa Rica Health Pass is a travel health certificate that must be obtained online before going to the country during the COVID-19 pandemic.

Once the passenger completes the electronic form, they will receive a QR code that serves as a Health Pass to enter Costa Rica.

This QR code must be presented to border authorities upon arrival at a Costa Rican airport.

It is important to understand that the Health Pass for Costa Rica is not a substitute for a visa.

Travelers will still need to obtain a visa if required to under Costa Rica’s visa policy.

How to Complete the Costa Rica Health Pass Online Registration?

To obtain a Health Pass, travelers must enter their details into the required fields of an online health declaration form application.

Completing the Costa Rica health pass online registration involves providing the following information:

  • Traveler’s personal information (name, nationality, etc.)
  • Passport details
  • Contact information
  • Travel information (date of arrival, mode of transport, etc.)
  • Vaccination status
  • Any COVID-19 symptoms the passenger is experiencing

By submitting the form, the traveler is declaring that all the information provided is true and accurate to the best of their knowledge.

Once this is done, the individual will receive a QR code by email. This can be downloaded to a mobile device or printed out on paper. It must be shown upon arrival in Costa Rica to enter the country.
