In response to the current COVID-19 pandemic, Chile has introduced an additional document for all visitors — the Chile Traveler’s Affidavit. 

Many other countries have also added a travel health form to their entry requirements. This type of document helps to strengthen borders measures during the coronavirus pandemic.

The Chile health declaration form must be completed by all international passengers who want to visit the country. Without an approved travel document, you will not be permitted entry.

What is the Chile Traveler's Affidavit?

The Traveler’s Affidavit is an entry policy that has been specifically introduced to help manage the COVID-19 situation in Chile.

The mandatory form is a type of Travel Health Certificate that shows officials that you are following the current protocol.

The Chile travel health form can be obtained online, and then shown to authorities during your journey, and upon arrival in Chile.

How does the travel health form work?

Specific information must be provided on the Chile Traveler’s Affidavit application form, which helps Chilean health authorities in different ways:

Track and trace positive cases

You are required to provide your contact details on the travel health form. This makes it easy for health officials to inform you if you have been in close contact with someone who has tested positive for coronavirus. 

If you are at risk, you will be contacted quickly to take the necessary steps, such as a test and quarantine measures.

Manage high-risk passengers

Current entry restrictions for Chile state that non-residents who have been in certain high-risk countries within 14 days of travel are not allowed to enter the country.

Before you can complete the application form for the Chile Traveler’s Affidavit, you must state if you have recently been in any of the high-risk countries. If you answer yes, you will not be allowed to continue with the application process.

Monitor COVID-19 entry requirements

During the application process for the Chile health declaration form, you must submit documents related to the COVID-19 entry requirements.

This way, the Chilean government can confirm that each passenger meets the current restrictions before they are granted entry into the country.

Who Needs to Complete the Traveler's Affidavit for Chile?

All international passengers arriving in Chile must have a completed travel health form ready to show to immigration officers.

If you plan to enter the country for any reason, and by air or sea, you must follow this policy.

Do children need to complete a travel health form?

Children who are under the age of 6 years old can be included as a dependent on an accompanying adult’s form.

To include children on your form, they must first be registered as a dependent on the homepage. They can then be added to your form in the dependents section.

Children over the age of 6 years old must have their own separate Chile health declaration form. A parent or guardian can help them to complete their application, or do it on their behalf.

Chile Health Declaration Form Requirements

Before starting the application process for the Chile Traveler’s Affidavit, you will need to have the following:

  • Contact details
  • Travel information
  • Accommodation address in Chile

In line with the current COVID-19 safety measures, you must also have all the following:

  • Negative COVID-19 PCR test result
  • Proof of full vaccination
  • Health insurance

Entry requirements change at short notice, so it is important to check the most recent travel restrictions before organizing your trip to Chile.

Chile Traveler’s Affidavit Online Registration

Before you can complete the travel health form, you need to create an account through the government website. When you have opened an account, you will be sent an email asking you to verify your identity.

You can open your online account at any point before you travel. You do not have to wait until you are ready to complete the application form, as your account will stay valid.

When you first access your account, you will be asked for your personal information. You need to provide:

  • Full name
  • Date of birth
  • Passport details

After completing your details, your account is then active, and you are free to apply for the Chile travel health form.

Chile Health Declaration Form Application

Once you are logged into your account, and have completed it with your personal details, you can start the Chile Traveler’s Affidavit application form.

The travel document only lasts for 48 hours, then it expires. This means that you can apply no more than 48 hours before you travel.

To start, you need to select ‘create new affidavit’. Before the form is available, you must declare if you have been in any of the listed high-risk countries within the last 14 days. If you have, you will not be allowed to proceed with the application form

When you have confirmed that you have not been in an area of high risk, the application form will open for you to fill in. You need to provide the following:

  • Accommodation address in Chile
  • Contact telephone number

You are then required to upload all supporting documents you have for the current COVID-19 entry requirements, including:

  • Negative PCR test
  • Proof of vaccination
  • Medical insurance

When all the required fields have been completed and the necessary documents attached, you can submit your application for the Chile health declaration form.

Once the form has been approved, it will be sent to you by email. It must then be printed and shown to airline staff and immigration officials. It is also advisable to have the digital copy on a mobile device.

  • Do I have to be vaccinated to travel to Chile?

    Yes, currently all international passengers entering Chile must be double vaccinated.

    A vaccination certificate must be uploaded with the Traveler’s Affidavit form before departure, and shown on arrival.

  • Does Chile have a mandatory quarantine rule?

    Residents of Chile who are not fully-vaccinated or have been in a high-risk country must undergo a 7-day quarantine.

    Non-residents cannot enter the country if they are not fully vaccinated or have been in a high-risk country.

  • Is the Chile Traveler’s Affidavit mandatory?

    The Chile travel health form is a mandatory requirement for all arrivals. Without a completed and approved copy of this travel document, you will not be allowed to enter the country.

  • Do I need a negative COVID-19 test result to enter Chile?

    Yes, all arrivals must have a negative COVID-19 PCR test result. Proof of this test must be uploaded during the application process for the Traveler’s Affidavit.

    Currently, only PCR tests are accepted. An antigen or other type of test will not be sufficient to enter.

  • Do children have to complete the Chile travel health form?

    All children over the age of 6 must have their own completed Traveler’s Affidavit for Chile.

    Children younger than 6 years of age can be included as a dependent on a parent or guardian’s form.
