Once you are logged into your account, and have completed it with your personal details, you can start the Chile Traveler’s Affidavit application form.
The travel document only lasts for 48 hours, then it expires. This means that you can apply no more than 48 hours before you travel.
To start, you need to select ‘create new affidavit’. Before the form is available, you must declare if you have been in any of the listed high-risk countries within the last 14 days. If you have, you will not be allowed to proceed with the application form
When you have confirmed that you have not been in an area of high risk, the application form will open for you to fill in. You need to provide the following:
- Accommodation address in Chile
- Contact telephone number
You are then required to upload all supporting documents you have for the current COVID-19 entry requirements, including:
- Negative PCR test
- Proof of vaccination
- Medical insurance
When all the required fields have been completed and the necessary documents attached, you can submit your application for the Chile health declaration form.
Once the form has been approved, it will be sent to you by email. It must then be printed and shown to airline staff and immigration officials. It is also advisable to have the digital copy on a mobile device.
