What does the travel health form do?

The Colombia travel health form has been introduced by the Colombian government to help protect border control officials and foreign travelers during the COVID-19 pandemic.

The purpose of the travel health document is to save time at border control, by reducing the time it takes to attend to each traveler.

The policy also helps to protect public health, by avoiding direct contact between immigration officers and passengers.

Showing the Colombia travel declaration form at check-in allows the airline to confirm that each passenger is following the current travel policy.

At border control, officials can ensure that all visitors crossing the border have abided by the mandatory entry requirements.

How is the information used?

The information collected gives health officials a clear record of the health status of each visitor coming into the country, to limit the risk of positive coronavirus cases entering.

The form also acts as a point of contact for authorities to easily communicate with passengers if they have been in close proximity to someone who has tested positive.

This way, visitors can be contacted as quickly as possible, and prevent the virus from spreading further.

Who Needs to Complete the Check-Mig for Colombia?

Any visitor entering Colombia from overseas is obliged to show a completed Check-Mig form.

Citizens of all countries must follow this COVID-19 policy, in order to be granted access into Colombia. This includes Colombian nationals and residents.

If you intend to go to Colombia for any reason, then you must follow this entry requirement. 

If you fail to comply with this policy in advance of travel, you will be obliged to complete the form on arrival. This can slow down the immigration process, and could even lead to you being refused entry.

The Check-Mig for Colombia also needs to be completed again to exit the country. A new form must be completed and shown when leaving.

Is anyone exempt from the Colombia travel declaration form?

There are no exemptions for the Check-Mig. All passengers must have this document, or risk bring refused entry into Colombia.

Before you start the application process, you should ensure that you have the following:

Currently, there are no other COVID-19 entry restrictions to travel to Colombia. The Check-Mig for Colombia is the only additional step that you need to take.

However, the situation is constantly changing, and all passengers should check the country’s current coronavirus travel restrictions before each trip.

If you attempt to travel to Colombia without checking the most recent travel requirements, you could risk arriving without the necessary documentation. This could prevent you from boarding your flight, or being permitted entry at immigration.

Check-Mig for Colombia Online Registration

You can complete the application for the Colombia travel health form using the online system. It is a simple process that only takes a few minutes of your time.

The application can be done within 48 hours of departure, and up to 1 hour before.

The program does not allow the fields to be completed unless the date of travel is within 48 hours, so it is not possible to do it any earlier than in the required time. Still, it is advisable to do your application as soon as you enter the 48-hour window, to avoid any possible delays.

Completing the health declaration form application

Once you enter the system, the form starts by asking you if you are entering or leaving Colombia. The Check-Mig is only valid for single-entry, so a separate form needs to be completed for each journey.

You will then be asked for your travel information. You need to provide details such as:

  • Mode of transport
  • Departure date
  • Travel provider
  • Flight number

Then you will need to give your contact details for when you are in Colombia, including:

  • Passport number
  • Phone number
  • Email address

In order to health screen each passenger, you also need to answer some questions about your current health status. These questions cover:

  • COVID-19 symptoms
  • Contact with someone showing symptoms
  • Testing
  • Any previous quarantine time

Once you have completed all the required fields and submitted your information, the application will then be processed. Within 24 hours, you will receive an email with your approved Check-Mig for Colombia.
