No one is exempt from completing the form. Any person who intends on crossing a border into India, for any reason, must present it on arrival.

If you are just planning on transiting through the country, the India Travel Health Form is still mandatory.

Do I Still Need a Visa if I have a Travel Health Form?

The Air Suvidha Self Declaration Form does not replace an Indian visa. 

The form is an extra measure specifically for COVID-19 control. Visa requirements for India remain the same.

Passengers need to apply for an India e-Visa as usual, and complete the health declaration form as an additional document.

India Health Declaration Form Requirements

Before you proceed to apply online for the travel certificate, it is important to first check that you are currently permitted to travel to India.

Due to the pandemic, travel restrictions are constantly changing, and entry into certain territories cannot be guaranteed.

When you have confirmed that you are able to travel to India, you can then complete the Air Suvidha Self Declaration Form.

Before you begin, you will need to ensure that you have the following:

As part of India’s safety measures against coronavirus, applicants are also required to provide proof of a negative test result from a COVID RT-PCR test. This test must be taken within 72 hours of departure.
